The Chatt Heals’ Community Health Partnership Initiative (CHPIn) is a community-centered engagement and implementation framework designed to augment traditional community health assessments, making them relevant to our rural communities.

While traditional assessments identify priorities, CHPIn helps communities organize around local priorities through continuous dialogue, shared responsibility, and collaborative action.
