The Chatt Heals’ Community Health Partnership Initiative (CHPIn) is a community-centered engagement and implementation framework designed to complement traditional community health assessments.

While assessments identify priorities, CHPIn helps communities organize around those priorities through continuous dialogue, shared responsibility, and collaborative action.
